Insurance and Payment Policies
Payment is expected at the time services are rendered. Services must be paid when received and a deposit of 50% is required on all orders. Payment for all materials, including glasses and contact lenses must be made in full prior to taking materials from the office. There are no exceptions.
Payment can be made with cash, check, VISA, Master Card, American Express, Discover or Care Credit.
We accept most insurance plans. Please contact our office by phone to inquire as to whether we participate with your insurance plan. The patient is responsible for all services rendered and materials purchased, regardless of insurance coverage.
