General information to get started
Patient Information Center
The following patient information is intended to assist you with your initial and follow up visits to Thomas Eye Center. Dr. Stuart Thomas, Dr. Kaj Thasen and the staff of Thomas Eye Center are dedicated to your overall health which includes your vision care. We utilize some of the most technologically advanced equipment available in the industry to diagnose and treat common vision problems. We recommend that you make an appointment to have your eyes checked annually however, if you are experiencing problems you should call our office immediately at (706) 549-7757.
We ask that you please arrive 15 to 20 minutes early for your appointment so you can browse our selection of frames prior to being dilated. If you must cancel your appointment, please give us at least 24 hours notice. Appointments cancelled with less than 24 hours notice will incur a $50.00 no-show fee.
Insurance & Payment
Payment is expected at the time services are rendered. Services must be paid when received and a deposit of 50% is required on all orders. Payment for all materials, including glasses and contact lenses must be made in full prior to taking materials from the office. There are no exceptions.
Payment can be made with cash, check, VISA, Master Card, American Express, Discover.
We also accept payments from healthcare financing option such as the healthcare credit card, Care Credit or the vision insurance plan, VSP. We accept most insurance plans. Please contact our office by phone to inquire as to whether we participate with your insurance plan. The patient is responsible for all services rendered and materials purchased, regardless of insurance coverage.
Patient Medical Information Form
The online Patient Medical Information form is a convenient way for our patients to update Medical History and Policy Notices at Thomas Eye Center. The online form is divided into eight short sections. After completing the form online, Thomas Eye Center will have a printout available when you visit our office so that you may verify, correct, or update any information at that time as well.
